What participants do

Participants map where they are now, name what is getting in the way, and build a plan they can hold in their hand. Visual aids and personalised attention address the ingrained behaviours that stop people reaching their own goals, so literacy never decides who can take part.

Closing the Gap Target 8 aims to lift the share of First Nations people aged 25โ€“64 in employment to 62 per cent by 2031. The Productivity Commission assesses the target as not on track, and employment is among the outcome areas recorded as worsening.

The defined model

Four stages, followed the same way every cohort, so participants are supported and enabled to make long-lasting change.

  1. In-depth first contact

    From the first meeting we find out everything: previous treatments and why they ended, education, and a comprehensive assessment of any offending and its severity where that is relevant.

  2. Detailed client profiling

    We get to know participants well enough to focus on the specific methods that will help them build better habits, mentoring and guiding them toward help and stability.

  3. Self-advocacy support

    Participants are taught to take ownership and speak for themselves, creating a practice they can carry into every other part of their lives.

  4. Gradual change in habits

    Change is built incrementally, so the positive shifts participants make are the kind that last past week six.

End-of-program checklist

  • Participant can name the concern in their own words.
  • Participant holds a plan with a first step and a date.
  • Case manager has the close-out summary and reporting extract.
  • Referral pathway agreed for anyone who needs more support.
